To apply to enter teams in The 2010  Banks and Brokers Football World Cup please download and complete the application form (download here) and email it to us here.
Please read the terms and conditions of entry before completing this form.

Please make sure you complete as many fields as possible, including the email details for all squad members. If any of your squad details are likely to change prior to the start of the Championship, please contact us to inform us of these changes giving us at least one week's notice.

It should be noted that each individual, company or organisation may apply to enter up to 4 teams in the Championhsip.

Entry fees are (£850 inc vat) per team.
